We recommend going to the "PATIENT FORMS" section on this page, printing off the paperwork, filling it out, and faxing or scan/emailing it back to us. This will help expedite your initial appointment without having to fill out all the patient information at our office, which we know can be time consuming.

Availability

We are committed to making ourselves completely available to you. If you need to be seen immediately, Dr. Carney and his staff will make every attempt to see you the same day. Some dental offices are only operating part time. Some offices are too busy to take the time to speak directly to their patients or referring doctors immediately. We at Pro Soft Implantology & Periodontics recognize and value your time, and know that having to wait to speak to the doctor or having to be called back later can be very frustrating. Dr. Carney will personally address each and every patient and referring doctor when needed. We believe that we can stop, take a moment, and address our patients' needs.

Patient Forms

In order to save you time on your first visit with us, please fill out the 2 the forms below either on your computer or by hand. When you are finished, simply print them out and bring them with you, or you can fax them to our office. Upon your arrival, we will verify that your information is correct and get you to see Dr. Carney on time.

 

Dental Insurance

In this ever-changing world of dental insurance, we can proudly say that we are on most the major PPO dental insurance networks.  Dr. Carney will work with you so that your visit and your services can be just as affordable as it is when using your dental insurance.

What many people do not understand is that dental insurance typically limits your annual benefit to about $1500 worth of TOTAL dental treatment, and it NEVER covers 100% of your dental treatment plan. In many cases, if we are not in your provider network, we will sometimes make bold accommodations with our fees for you and your referring dentist so that we don't have to file "OUT of NETWORK." By doing this, we are able to minimize dental insurance involvement in the process, and allow you to use your dental benefits for other treatment as needed with other providers. It is ultimately a "win-win" situation for both you the patient and us, the dental practice. When you visit our practice, we will be happy to go over this with you in greater detail as we develop your treatment plan and evaluate the necessary fees.
